NATIONAL PROGRAM FOR DENGUE CONTROL

IN SOUTHERN VIETNAM

 

I/ HISTORY

Dengue Fever/Dengue Hemorrhagic Fever (DF/DHF) is a vector-borne disease caused by dengue virus.

There were dengue fever outbreak in southern Vietnam throughout the year. Before 1998, there has been about 50,000 Dengue cases every year with nearly 200 death cases, mainly in children counted more than 90% cases. In 1998, an dengue outbreak year, there were 124,000 cases and 350 death cases in southern Vietnam. In order to reduce morbidity and mortality of DF/DHF, the National Program For Dengue Control was established in 1999.

 

II/ OBJECTIVES

  1. Main Objectives
    • Mortality reduction
    • Morbidity reduction
    • Outbreak epidemic control
    • Socializing and establishing legislation in  Dengue control.
  2. Specific  Objectives:

·         Decreasing 10% of Cases Fatality Rate in comparing to the average of 1996 – 2000.

·         Decreasing 10% of Morbidity/105 population in comparing to the average of 1996 – 2000

·         Confirming 10% DHF cases by serological test and 3% by viral isolation

·         Managing 80% DHF cases (grade I, II) in health service and in community

·         Investigating vector index in 100% sentinel districts and 50% remains

·         Establishing health collaborator network for source reduction in 10%-15% communes in each provinces

·         Signing agreement in source reduction with 100% householders in sentinel communes and cleaning larvae in 50% households

·         Implementing source reduction campaigns at least twice per year in 100% sentinel communes

 

III/ ORGANIZATION

  1. Regional level

Establishing steering committee and subcommittees for case management, epidermiology surveillance, entomology surveillance, serology surveillance and epidemic prevention for:  

    • Training about case management and outbreak response skills for health staffs in lower levels
    • Surveillance DF/DHF situation for timely epidemic forecasting
    • Monitoring lower levels for modifying dengue control strategy and activities
    • Studying an effectiveness dengue control model
  1. Local level

Establishing for DF/DHF prevention and control in different levels:

·         Collaborating between government, health service, education and other sectors

·         Putting the Dengue control objectives into ordinance in socio-economic development plans in local area

·         Raising the responsibility of community through agreement of “No Larvae In House” between householders and the People’s committee of communes

·         Closing collaboration between sectors of treatment, prevention, laboratory at all levels

 

III/ ACTIVITIES

1. Epidemiological surveillance

            Diseases surveillance

·         Detecting DF/DHF case from the lowest level, i.e. commune health service

·         Reporting cases according national guideline: timely, exactly and fully

·         Carrying-out virus isolation and serological test for viral infection cases and DF/DHF cases during year, especially on dry season

            Vector surveillance: Implementing vector surveillance in sentinel points where represent ecological environment in local for:

·         Monitoring vector situation

·         Evaluating effectiveness of dengue control 

2. Activities of Mortality reduction

            Principle: “Early Detection, Correct Treatment” for reducing CFR.

·         Training to improve DF/DHF cases management

·         Providing essential equipment for diagnosis, e.g. Hematocrite machine, microscope, supporting for diagnosis at district hospitals and commune health centers

·         Health education about knowledge of DF/DHF detection and management with slogan “Straight To Health Services When Get Fever” through health collaborators network and mass media
